Finding: The right posterolateral PZ lesion has a broad interface with the PZ, a spherical shape, and an internal T2-hyperintense focus. The findings suggest an extruded BPH nodule, extending from the TZ into the PZ. Such nodules will exhibit decreased ADC and early enhancement, thus mimicking tumor. According to PI-RADS v2, lesions suggestive of an extruded BPH nodule may be assigned a DWI score of 2, thus receiving an overall PI-RADS category of 2.
