Finding: No focal abnormality is identified on multi-parametric imaging. The posteromedial base demonstrates a crescentic shaped structure (solid arrows) with decreased T2 signal and decreased ADC, surrounding the ejaculatory ducts (dotted arrows). This represents the central zone, a normal anatomic structure that is distinct from the transition zone. It is important to recognize the central zone as a normal structure, so as not to mistake it for a large suspicious lesion. A subsequent standard systematic biopsy was performed.
